We use an advanced Electronic Medical Records system called Elation. The member's interface is called "Passport". Members can access their health information and send or receive emails from Dr. Hussain or her staff.

• If you have not signed up and you want to sign up to Passport, please send an email to member_access@seniorwellnesscenter.org indicating your name and your will to sign up and we will send you the sign up invitation or call the office and we will help you through the process.
